Shipment of human remains

SHIPMENT OF HUMAN REMAINS

For the shipment of human remains from the United States to Bosnia and Herzegovina, it is necessary to submit the following documents:

1. A formal request for shipment (the request is done by the family or by the Funeral Home Director)

2. A completed form  required by the Ministry of Health in Bosnia and Herzegovina (provide one out of three forms depending on the location of the burial: Federation of Bosnia and Herzegovina, Republika of Srpska, Brčko district BiH).

3. Original Death Certificate or a notarized copy;

4. An original or a notarized letter confirming that there are no contagious diseases present from the Health Department;

5. An original or a notarized copy of an embalmer’s affidavits or cremation certificate (if a cremation is in question);

6. A letter from the funeral home from Bosnia and Herzegovina (obtained by the family) for the proposed burial

7. Valid passport of Bosnia and Herzegovina of the deceased

8. Letter from the Funeral Home stating the flight itinerary

9. Notarized letter from the Funeral Home director certifying that the remains have been prepared according to international rules and regulations concerning the transport of human/exhumed remains and that the casket/urn contains exclusively the remains of the deceased

10. Cashier’s check or a money order for the required amount, >price list<

IMPORTANT: Uncremated Remains: Must be placed in a hermetically sealed and metal-lined casket. The casket must be then placed in an airtray. Cremated Remains: Must be transported in funeral urns which are sealed and securely protected from damage by cushioned packaging.
