Vital Records Registration


 

Determination of Individual Identification Number for citizens who reside abroad

In order to determine an individual personal identification number, it is necessary to provide the following documents:

1. A completed and signed application request (request for individual identification number), an original birth certificate from BiH (cannot be older than 6 months from the day issued).

2. Original certificate of citizenship of Bosnia and Herzegovina (cannot be older than 6 months from the day issued). NOTE: This document is not required when registering the birth of a child.

3. Cashier’s check or money order for the required amount, >price list<



REGISTRATION OF THE BIRTH OF A CHILD:

Along with both the request for determining an Individual Personal Identification Number and the  form for registering the birth of a child, you are obligated to provide the following:

1. International birth certificate of the child or an original birth certificate verified with an "Apostille seal" from the Secretary of State’s office in the state where the child was born. This document must not be older than six (6) months.  It must also be translated into one of the official languages of Bosnia and Herzegovina, certified by a qualifed interpreter.

2. Proof of parents’ citizenship of Bosnia and Herzegovina, for which one of the following can be provided:

a.) copy of a valid travel document i.e. passport

b.) birth certificate and a certificate of citizenship, both can not be older than six (6) months.

3. Cashier’s check or money order for the required amount >price list<


Procedure for subsequent registration

1. Completion of appropriate document in legible print

2. Original international birth certificate.

REGISTRATION OF MARRIAGE:

Citizens who have married outside of Bosnia and Herzegovina and wish to register their marriage at the registrar’s office in Bosnia and Herzegovina, by providing the following:

1. Original marriage certificate issued by the official authorities of the country where the ceremony took place, which must also be certified with an “APOSTILLE” seal from the appropriate authorities of the state (in the United States, internationally recognized certification is performed by the office of the Secretary of State).

(Note: Marriage law of Bosnia and Herzegovina recognizes only civil marriages or marriages concluded before the civil authorities).

2. Completed “Registration of marriage” form.  If the form has not been obtained, it can be sent to you, upon your request, by mail or fax.

3. Certified translation of the marriage certificate, translated by appropriate personnel.

4. Proof of citizenship of Bosnia and Herzegovina (copy of Bosnian-Herzegovinian passport or an original certificate of citizenship of Bosnia and Herzegovina, which cannot be older than 6 months).

5. Cashier’s check or money order for the required amount, >price list<

NOTE: The Consulate General of Bosnia and Herzegovina in Chicago will be unable to process a request for issuing a passport to include the new last name to individuals who, along with the request for issuing a travel document, have not submitted a completed marriage registration form.



REGISTRATION OF DEATH:

Along with the request for registration of death, you are obligated to provide the following:

1. Original or certified copy of the death certificate issued by the foreign agency abroad. The certificate must be verified with an "APOSTILLE" seal from the Secretary of State’s office in the state where the death certificate was issued. The death certificate must be translated into one of the official languages of Bosnia and Herzegovina, verified by a court interpreter or authorized translator.

2. Copy of documents that prove the identity and citizenship status of the deceased person.
